Supportive Services Provided by Caregivers

Instrumental Activities of Daily Living (IADLs)

Caregivers assist with tasks that ensure smooth daily functioning, such as:

  • Household Management: Laundry, shopping, housekeeping, meal preparation, and cleanup.
  • Transportation: Accompanying the patient to medical appointments or other necessary outings.
  • Equipment Maintenance: Care and upkeep of wheelchairs and adaptive devices.
  • ​Medication Management: Ensuring timely and correct medication use.





OR

Activities of Daily Living (ADLs):

Hands-on assistance or supervision in areas such as:

  • Eating
  • ​​Toileting
  • ​Dressing
  • ​Bathing
  • ​Transferring
  • ​Mobility/Ambulation

Onboarding Process for Clients

Step 1: Initial Assessment

A Registered Nurse visits the patient's home to conduct a comprehensive assessment.

Step 2: Paperwork Submission

Documentation is sent to the patient's primary care provider for review and signature.

Step 3: MassHealth Approval

Once paperwork is signed, it is submitted to MassHealth for program approval.

Step 4: Caregiver Recruitment

Upon approval, a relative caregiver is hired to start providing care.

Step 5: Ongoing Support

Nurses or social workers visit the patient's home monthly or every 60 days to provide support and
oversight
